Report it
- 1Workers: in the worker app, choose Report Incident (or from your shift screen). Pick what happened, answer the reportable question, describe facts and immediate actions.
- 2Office: log it under Incidents in the dashboard, or via the incident report form under Workers.
- 3Every incident routes to the incident register automatically, with follow-up actions created for the case manager.
Is it NDIS reportable? The legal test
When you flag an incident as NDIS reportable, Parkways shows the statutory breakdown before accepting it. An incident is reportable ONLY if both parts hold:
- 1It is one of the six categories in s73Z(4) of the NDIS Act: death of a person with disability; serious injury; abuse or neglect; unlawful sexual or physical contact or assault; sexual misconduct including grooming; or unauthorised use of a restrictive practice.
- 2AND it happened in connection with the supports we provide: during support delivery, arising out of it, or because of how supports were provided, altered or withdrawn.
Common over-reports that are NOT reportable
- 1Medication errors, including missed S8 medications, with no serious harm.
- 2A participant being aggressive toward a worker (record it internally, it is a WHS matter).
- 3Coincidental events unconnected to our support delivery.
- 4Property damage, near misses, or hospitalisation for an unrelated illness.
- 5Incidents you witnessed in another provider's service.
If it IS reportable
Call Nicholas Dodemaide, Director and Incident Manager, on 0400 378 009 immediately. The 24 hour window to notify the NDIS Commission starts when any of us becomes aware, and only the authorised approver can submit the notification through the Commission portal. Unauthorised restrictive practice without harm has 5 business days instead of 24 hours.
Phone first, paperwork second. Keep documenting in Parkways after the call while details are fresh. The incident page shows a live countdown until it is marked reported to the Commission.
